The Toronto General Hospital Training School for Nurses, later also referred to as School for Nurses and School of Nursing, was established in April 1881 and formally dissolved in August 1974.
The Training School for Nurses at the TGH was established in April 1881 at the instigation of the TGH Lady Superintendent Harriet Goldie and with the approval of the TGH Board of Trustees (granted March 21, 1881). The School’s initial enrolment was 17 students, but of these eight dropped out in the first seven months. The first graduating class (5 students) completed their program in March 1883. Initially, a panel of doctors examined graduating nurses orally; later, formal written examinations were instituted.
Miss Goldie retired in 1883 and was briefly succeeded as TGH Lady Superintendent and Superintendent of the Training School by Eliza Storrie Fulford, and then in March 1884 by Lucy Pickett. When Miss Pickett resigned in late 1884, Mary Agnes Snively was appointed in her place.
The first nurses’ residence was opened in 1887 and in 1896 the education programme was increased from two to three years. In 1905 Miss Snively’s dual role as Lady Superintendent and Superintendent of the Training School was divided, allowing her to concentrate on administering the School.
In 1907 the first arrangement of inter-institutional affiliation for the purposes of providing student nurses with specialized training was established between the TGH Training School for Nurses and the Hospital for Sick Children (TGH students received instruction and experience in obstetrical and medical nursing).
Miss Snively resigned in 1909 and in 1910 Robina L. Stewart was appointed as Superintendent of the Training School. Miss Stewart resigned in August 1913 and was succeeded in September 1913 by Jean I. Gunn.
In 1921 the education program in place at the Training School encompassed 630 hours of instruction:
Anatomy and Physiology, 90; Chemistry, 32; Bacteriology and pathology, 16; Hygiene, 12; Dietetics, 22; Materia Medica, 48; Bandaging, 20; Massage, 12; General and Medical Diseases, 36; Surgical and Gynaecological Diseases, 36; Obstetrics, 24; Pediatrics, 14; Communicable Diseases, 10; Nervous and Mental Diseases, 8; Diseases of Special senses, 7; Nursing procedures and Theory, 172; Ethics and History of Nursing, 28; Public Health and Social Services, 43 (“History of the School for Nurses” 1931, p. 39. TH 1.1.9.2).
In the 1920s and 1930s the School began admitting affiliated students from other schools of nursing: in 1928 students from the Wellesley Hospital School of Nursing for experience in medical nursing; in 1931 students from the Ontario Hospital for Mental Diseases at Whitby for surgical, medical and obstetrical nursing; and also in 1931 students from Riverdale Hospital in Toronto for experience in emergency nursing service. By 1931, its 50th anniversary, the School had graduated 1,867 nurses.
Jean Gunn retired in 1941 and in 1942 Mary Macfarland was appointed Superintendent of Nurses and Director of the School. In 1950 the name of the school was changed to the TGH School of Nursing and Miss Macfarland was designated Director of Nursing (in charge of both the Department of Nursing and the School of Nursing) assisted by an Assistant Director of Nursing Education and an Assistant Director of Nursing Services.
In 1956 the education program was fundamentally changed, from three years of instruction to two years’ instruction plus a one-year internship for professional experience.
Miss Macfarland retired in 1960 and was succeeded as Director of Nursing by Jean Dodds. In 1968 the curriculum was again changed: the third, internship year was dropped, leaving a two-year academic program In 1970 Jean Dodds retired and was replaced as Director of Nursing by Patricia Stanojevic. In 1974 the TGH School of Nursing ceased to function as an independent institution as a consequence of the transfer of diploma nursing education in Ontario from hospital schools of nursing to the community college system The TGH School of Nursing was subsumed as a campus of the George Brown College School of Nursing.

Series consists of the permanent student academic record files created and maintained by the TGH School of Nursing. Records contained in individual files vary according to the date of creation. Most files will include the student history card, which contains grades by term as well as by subject; hours of practical work by department; dates and duration of absenteeism along with a list of cause or diagnosis and whether the time was made up; date of birth; home address; religion; place of birth; nationality of parents; instructors’ assessments; and dates of admission to the School and completion of courses. Information about vacations or illnesses was collected for the purpose of calculating the number of days absent and ensuring that the time was made up. Some student history cards include an attached picture. Files may also contain certificates of preliminary education (high school academic records); reports including records for hours of nursing care from affiliated Hospitals (e.g. The Hospital for Sick Children or the Toronto Psychiatric Hospital), schools of nursing, and/ or education programs which students attended to receive specialized training and experience; copies of medical records; and photographs. Some files also contain materials subsequently added, including letters from students or their agents requesting letters of reference for employment or information contained in their files, the actual letters of reference, applications for employment in the Nursing Department of the TGH and employment records, newspaper clippings and general correspondence.

Sub-series consists of reports prepared by the Superintendent/ Director of the TGH School of Nursing and presented at the School’s annual convocation ceremonies for graduating students in May or June and/or submitted to the TGH Superintendent/ Executive Director as an academic year-end report, usually in September. Graduation reports 1 to 7 and 12, for the years 1882 to 1888, and 1893 are missing. For each of the years 1889 to 1906 a single report was prepared and was presented at graduation. For each of the years 1907-1921, 1923-1925 two reports were prepared: the first was presented at graduation in May; the second was submitted to the TGH Superintendent. For each of the years 1926 to 1970 a single report was prepared and was presented at graduation. For each of the years 1971 to 1973 a single report was prepared and was submitted to the TGH Executive Director.
Reports contain general information pertinent to the administration of the School, including statistics re: staff, applicants, admissions, and graduations; developments in the School’s curriculum and facilities; scholarships and awards; and information about staff appointments.

Reports for 1960 to June 1970 and for September to December 1970 are missing.
Reports contain general information pertinent to the administration of the School, including statistics re: staff, applicants, admissions, and graduations; developments in the School’s curriculum and facilities; scholarships and awards; and information about staff appointments. Some reports also contain information about individual students, including information pertaining to academic performance, days lost to illness, infractions against School regulations and penalties imposed by School administrators.
Reports for 1905 to 1947, and 1970-1973 focus exclusively on the School of Nursing. Reports for 1948 to 1959 also contain information pertaining to the administration of the TGH Department of Nursing, including lists of departmental/ward head nurses (1951-1959) and recovery room reports (1952-1959).

Sub-series consists of annual reports prepared by the Superintendent/ Director of the TGH School of Nursing and submitted to the Nursing Branch, Ontario Department of Health (1930-1962/1963) and the College of Nurses of Ontario (1963/1964-1969/1970). Reports were required for accreditation as an approved school for training registered nurses in Ontario. Report for 1946 is missing.
Reports contain information about the School’s faculty (including academic credentials and professional experience), administration, students (numbers admitted, graduated, resigned etc.), curriculum (courses offered, hours of instruction provided), professional experience provided for/ required of students (hours and types of nursing service provided by students), and facilities (classrooms, laboratories, library, residence).

Sub-series consists of reports prepared by the Director of the TGH School of Nursing and submitted monthly (May 1946-1958), then quarterly (1959-1962) to the Ontario Department of Health.
Reports contain information about registered nurses employed at the TGH and about student nurses enrolled in the School; information about recent changes (appointments, resignations etc.) in the School’s administration and teaching staff and in the Hospital’s Department of Nursing; and information about students, including dates of birth, student illnesses and lists of recent withdrawals, graduations, and admissions.

Sub-series consists of annual reports prepared by the Superintendent of the TGH School of Nursing and submitted to the New York State Education Department, University of the State of New York at Albany. The reports were required as a condition of the School’s continuing accreditation by the New York State Education Department.

File 1.1.5.5 is a register of students’ attendance 1927-1936. Files 1.1.5.6 and 1.1.5.7 are registers of probationary students 1913-1925 and 1926-1943, including students’ names, age, address; dates of probationary and full admission to the School; notes on students’ health, character, academic performance and potential. Files 1.1.5.8 and 1.1.5.9 are registers 1948-1958 and 1958-1970 documenting use of the School seal, including date, description of purpose of use, and signature of authorized user.

Series consists of nurses’ day books jointly maintained by the TGH School of Nursing and Department of Nursing. Files 1.1.6.1 to 1.1.6.3 contain a daily log of service performed by TGH nurse administrators, head nurses, dietitians, graduate nurses and student nurses, including names; location (department, ward, building) of service; notes on vacations, leaves of absence, illness and disciplinary measures imposed for misconduct. File 1.1.6.4 is a daily log of service performed in the TGH Burnside Obstetrical Wing by TGH School of Nursing students and students from affiliated schools of nursing, including names, TGH School of Nursing class year or affiliated school, ward/floor and hours worked each day, and date of course completion.

Series consists of various materials documenting ceremonies and special events associated with the TGH School of Nursing. File 1.1.9.1 contains a commemorative publication, prepared by the TGH School of Nursing Alumnae Association to honor the graduates of the School who served during World War I.
File 1.1.9.2 contains materials relating to the School’s 50th anniversary celebrations in 1931, including a program of events, a list of letters received, a narrative summary of principal events, census of registration, text of address by Mary Snively, a list of donors of prizes and scholarships for the School 1900-1931, a list of living graduates of the School 1883-1930 with their address and place of occupation in 1931, a History of the School for Nurses published by the TGH School of Nursing Alumnae Association, and correspondence relating to the publication of the History. File 1.1.9.20 contains a copy of the second volume of the History, published in 1968 by Mary E. Macfarland, former TGH Director of Nursing, and intended to mark the 85th anniversary of the founding of the School, April 1966.
File 1.1.9.10 contains copies of invitations to the School’s graduation dinner for 1958, 1959, 1963 and 1967. Series consists of 8 binders of course curriculum and 1 record book, which were maintained by the Department of Nursing Education along with student transcripts. File 1.1.13.1 consists of an oversized record book containing the examination grades for all students from the class of 1961 until the class of 1970, organized by class and then by courses within each year. The examination records for the Hospital for Sick Children Medical Surgical Nursing Lectures and Hospital for Sick Children Obstetrical Nursing course, for the same time period, are also included in this volume. Courses for which curriculum is provided include Philosophy of Nursing, Nursing I-III, Paediatric Nursing, and Psychiatric Nursing. Some of the binders contain a copy of the final examination, and class lists including individual addresses and marks. A more extensive course and curriculum list is provided for years 1970-1974.
The records in this series were transferred to the Archives in June 2006 by the Coordinator of Nursing Education.

In 1950 the Toronto Western Hospital School of Nursing was renamed the Atkinson School of Nursing of the Toronto Western Hospital when a grant was received from the Atkinson Foundation for Nursing Education. At the same time the School became independent of the Toronto Western Hospital's Department of Nursing with the appointment of a Director of Nursing Service to head the Department, while Miss Gladys J. Sharpe became Director of Nursing Education, in charge of the School.
From 1950-1955 a pilot program was carried out at the School, by which the education program was reduced from three years to two years and a one-year internship. In 1955 this change was adopted permanently.
Following Miss Sharpe’s retirement in 1951, Blanche Duncanson (1951-1958) and Amy Griffin (1958 - ?) served as Director of Nursing Education. Beginning in 1972 the Atkinson School of Nursing was gradually phased out of independent existence, and in 1974 the School was formally dissolved and its functions transferred to George Brown College.

The Grace Hospital Training School for Nurses originated as the Training School for Nurses for the Toronto Homeopathic Hospital in 1890. In 1936 the process of amalgamation linking Grace Hospital with the Toronto Western Hospital (TWH) was completed and the TWH School of Nursing subsumed the functions of the Grace Hospital Training School for Nurses.
Miss Georgie L. Rowan served as Director of the Grace Hospital Training School for Nurses 1911-1936. Following the dissolution of both Grace Hospital and the Training School for Nurses in 1936, Miss Rowan became Supervisor of Nursing for the Private Patients Pavilion of the TWH until retiring in 1943.

The Toronto Hospital’s Department of Public and Community Affairs was established in 1986, replacing the separate Public Relations Departments of the Toronto General Hospital and the Toronto Western Hospital. David Allen, formerly Director of Public Relations at the TGH, headed the Department from 1986-January 1998 as Vice President, Public and Community Affairs. In January 1998 the name of the Department was changed to Department of Public Affairs and Communications. Gloria Bishop (1998) served as Vice President, Public Affairs and Communications, reporting directly to the President.
The Department prepares and publishes the TTH staff newsletter and the Hospital’s annual reports; coordinates TTH fundraising programs; manages public events and ceremonies involving TTH; authorizes and supervises all internal Hospital information postings/ distributions; coordinates and manages media relations for the Hospital; and responds to requests from media and the public for information about the Hospital and its programs, services and facilities.

Series consists of photographs taken and collected by the Department during the time of David Allen as part of the Public Relations Department’s Photo and Slide Library. Files include photographs of Administration (former and current) of The Toronto Hospital, Trustees, Auxiliary, Dignitaries and Celebrities, Medical Staff, Residents, and so-called “historical archive files” referring to photographs of buildings, sites and past events. Some of the files also include newspaper clippings, enlargement and usage notations, speeches or programs, cataloguing information, consent forms, and mock-ups or samples of publications for which the photographs were used. With the various changes in management structure since the time of David Allen, records were originally kept in the office of David Occipinti, A/V Technician. In anticipation of the demolition of the Bell Wing, the file cabinets of photographs were passed onto Kathryn Rumbold, Manager of the Artifact Collection, and then eventually to a storage room in the administrative buildings (R. Fraser Elliot), before being transferred to the Archives. Some photographs from the University Health Network era (post-1999) seem to have been inserted into this series. Additional files not in the original file list have also been added, some out of order.

File 2.8.1 includes 2 photographs of Nuclear Cardiology from the early 1980s.
File 2.8.2 contains a photograph of a construction site at TGH in July 1977 and a patient handbook from TGH from [198-?].
Files 2.8.10 to 2.8.13 were originally kept in one binder and probably belonged to Peter Edwards, who was at one point Chairman of the Historical Committee. File names are taken from the divider titles.
File 2.8.16 includes a guide for the della Robia style plaques which hung in the Outpatient Clinical in the Mulock/Larkin Wing of the Toronto General Hospital.
File 2.8.30 includes copies of photographs from the 1940s that were made in 1978.

The Ontario Cancer Institute was established in 1952 by an Act of the Ontario Legislature.
Bill 172, an Act to amalgamate The Toronto Hospital and the Ontario Cancer Institute / Princess Margaret Hospital came into effect on January 1, 1998. The departments consolidated but OCI/PMH was to retain its own identity. The combined oncology program became Ontario Cancer Institute / Princess Margaret Hospital a division of The Toronto Hospital.
The original mandate for the Institute was to plan, construct, and establish buildings to accommodate cancer research, diagnosis and treatment, and the observation of and consultation with cancer sufferers. This mandate was broadened when the Institute was reconstituted by the Cancer Act, 1957, "to maintain, manage and operate a provincial hospital with facilities for cancer research, diagnosis and treatment". Construction at the 500 Sherbourne Street location began in 1954 and the Hospital opened on May 1, 1958 with 87 beds.

The scientists in the Research Division form the basis of the University's Medical Biophysics Division. The hospital was responsible to the Institute's Board which was made up of representatives from The Ontario Cancer Treatment and Research Foundation, the University of Toronto and its teaching hospitals.
The Ontario Cancer Institute was originally organized into four divisions under the authority of the Institute's Director. These divisions were: clinical services, hospital services, physics, and biological research. The hospital's active and consulting medical staff were grouped under the umbrella of clinical services, while both patient services and administration were grouped under hospital services. The administrative structure of the hospital has undergone various changes over the years. Notably, by 1969, the Division of Hospital Services had been dismantled , and its various departments had been placed under the heading of Administrative Staff.
To mark the visit in 1958 of Her Royal Highness, Princess Margaret, the Government of Ontario named the new hospital "The Princess Margaret Hospital". The hospital is commonly known as the Ontario Cancer Institute/Princess Margaret Hospital (OCI/PMH).
As of November 13, 1995, the Hospital officially moved to 610 University Avenue, and the new hospital facility was recognized by a visit from Her Royal Highness, Princess Margaret, on July 12, 1996. OCI/PMH amalgamated with The Toronto Hospital on January 1, 1998.

4.2.1 Merger agreement between TTH and OCI/PMH (1997)
The integration of the Finance Departments of the Toronto General and Toronto Western sites of The Toronto Hospital (TTH) was completed in 1986. This involved the integrated management of the Payroll, Research accounting, and Foundation accounting departments, as well as the adoption of common reporting and budgeting methodologies.
The Finance Directorate’s mission was:
<blockquote>“…to provide efficient, timely, high quality accounting, fiduciary, financial reporting, analysis and other services that, in collaboration with other directorates, would enable the [Toronto Hospital] to achieve its overall mission of providing exemplary patient care and fostering excellence in health care delivery, teaching and research.”</blockquote>
The first Vice-President, Finance was Mr. J. Elliott (1986-1991), previously the Director of Finance for the Toronto General Hospital. The position of Vice-President, Finance was subsequently held by Ms. D. Arsenault (1992-1993), Mr. John A.B. Callum (1993-1997), and Ms. Susan A. Connor (1998- ). By 1997 the Vice-President, Finance was also Chief Financial Officer.

Series consists of a bound volume of the Daily Record of the Operations of the Toronto Western Hospital from 1962-1981 and patient admission ledgers, also from the Toronto Western Hospital. Daily Records of Operations of the Toronto Western Hospital include the numbers of inpatients (adult male, adult female and infants), numbers admitted or births during the day, numbers of discharged during the day and numbers of deaths. The earlier patient admission ledgers include names of patients, their procedures, names of the surgeons and anaesthetists, and clinical information such as blood pressure. Later patient admission ledgers include registration numbers, name of patient, age, sex, date of admission and name of attending physician. File 5.5.8 is a computer printout that includes the additional information of medical record number and admitting diagnosis.

While the formal merger between Toronto General Hospital and Toronto Western Hospital officially came into effect in 1986, it was 1988/89 before single leadership positions had been created for all of the surgical divisions in The Toronto Hospital. During the three-year transition period both the Toronto General Division and the Toronto Western Division maintained a Department of Surgery.
Between 1986 and 1988/89, the Department of Surgery at the Toronto General Division consisted of the following 7 divisions: Cardiovascular Surgery, General Surgery, Neurosurgery, Orthopaedics, Plastic Surgery, Thoracic Surgery, and Urology. The Department of Surgery at the Toronto Western Division consisted of the following 6 divisions: Cardiovascular Surgery, General Surgery, Neurosurgery, Orthopaedic Surgery, Plastic Surgery, and Urological Surgery.
In 1988/89 the James Wallace McCutcheon Chair in Surgery was established. The Surgical Directorate, headed by the Surgeon-in-Chief (also known as Vice President Surgical Directorate and James Wallace McCutcheon Chair), was responsible for the administration of the TTH Department of Surgery and various clinical departments including Anaesthesia, Gynecology, and Ophthalmology, as well as the Peter Munk Cardiac Centre and the Neurosciences Priority Program By 1998 the responsibilities of the Vice-President of the Surgical Directorate also included overseeing the departments of Bioethics, Dentistry, ICU, Operating Room-PACU, and Women’s Health. The Surgeon-in-Chief reported to the Executive Vice President and Chief Operating Officer.
The position of McCutcheon Chair, Surgeon-in-Chief and Vice President Surgical Services has been held by Dr. Alan R. Hudson (1989-1991) and Dr. Paul Walker (1992-1999).

Series consists of two separate accessions of material related to the Toronto General Hospital and accumulated by the Office of the Surgeon-in-Chief.
Files 6.1.1 to 6.1.2 consist of a half-filled bound volume of signatures spanning the time period of April 5, 1935 to January 16, 1959, and an accompanying 5 page description of its history by Ronald J. Baird, Professor of Surgery, University of Toronto, and Senior Cardiovascular Surgeon at The Toronto Hospital. The book of signatures is a combination visitors’ register/attendance book with groups of signatures that do not always indicate the occasion. Some of the occasions noted include post-graduate courses and club meetings, such as for the Interurban Orthopaedic Club, McGill Toronto Surgical Club, and the Cardiovascular Surgeons Club. Notable signatories include Evarts A. Graham, Charles H. Best, Gordon Murray, Frederick G. Banting, Wilder Penfield, and Norman Bethune. Regarding the first pages of this volume, Dr. Baird remarks in his description, “what a galaxie [sic] of future stars in Toronto Surgery!” Dr. Baird’s description gives extensive details as to how the volume came to be in his possession before being passed onto Surgeon-in-Chief, Dr. Paul Walker.
Files 6.1.3 to 6.1.5 consist of various agreements and indentures that were originally kept in one file folder. The file had originally been in the possession of Dr. Alan Hudson, McCutcheon Chair and Surgeon-in-Chief at The Toronto Hospital from 1989-1991, and President and Chief Executive Officer, from 1991-2000. The file had been passed onto his successor Dr. Paul Walker, Vice President Surgical Directorate and Surgeon-in-Chief of The Toronto Hospital during a transfer of office either when Dr. Hudson became President and Chief Executive Officer (1991) or when Dr. Paul Walker resigned as the James McCutcheon Chair and Surgeon-in-Chief (March 31, 1999) to become Vice-President of the Toronto General Hospital (April 1, 1999). Subsequently the file came to be held by Dr. Walker’s successor as McCutcheon Chair in Surgery, Surgeon-in-Chief & Director of Surgical Services at University Health Network, Dr. Bryce Taylor (April 1, 1999- ).

The medical administration of The Toronto Hospital (TTH) is the responsibility of the Medical Advisory Committee (MAC), also referred to as the Medical Advisory Board (MAB). The MAB of TTH succeeded the MAB of the Toronto General Hospital when TGH merged with the Toronto Western Hospital in 1986 to form The Toronto Hospital.
Dr. P. Garfinkel (1986-1989), Dr. W. McIlroy (1989-1991), Dr. W. Francombe (1991-1996) and Dr. John R. Wright (1996- ) have served as TTH MAC Chair.
The Medical Advisory Board (MAB) of the Toronto General Hospital (TGH) was established in 1876, under the authority of the Medical Superintendent and was responsible for the administration of all medical services and facilities at the hospital until the TGH’s merger with the Toronto Western Hospital in 1986 to form The Toronto Hospital (TTH). Following the merger, the MAB of the TGH was succeeded by the MAB of the TTH.
From time to time the MAB of the TGH created various standing committees with responsibility for fulfilling specific functions related to the medical management of the hospital.
Although its composition frequently changed, the MAB of the TGH was generally composed of the heads of the various medical, surgical and clinical departments, with additional members drawn from these and other hospital departments. The TGH chief executive officer was an ex-officio member of the MAB.

The Ambulatory Care Planning Committee was formed in 1989 to develop long-range planning for the construction of the TWH Ambulatory Centre. Dr. Charles Hollenberg served as the first chair (1989-199-?).
The Ambulatory Care Committee was created by the Medical Advisory Board in 1969 to replace the Emergency Committee and the University Clinics Committee. The Ambulatory Care Committee was to oversee all ambulatory care affairs in the hospital with membership providing representation from the various departments and services involved in ambulatory care. The Committee discussed matters such as care for patients in the emergency department of the hospitals and the administration of university and community health clinics. Dr. D.A. Stinson (1969-19-?) served as the first chair of the committee.
In 1976 the Medical Advisory Board created an Ad Hoc Committee for Ambulatory Planning which was to study the detailed planning for an Ambulatory Care facility. Dr. W.S. Goodman served as first chair of the Committee (1976-19-?).

As of the meeting of October 1986, a joint meeting of the Boards of both the Toronto General Hospital and the Toronto Western Hospital, the minutes change from being for the TGH Board of Trustees to those of the TTH Board of Trustees. It was at that meeting that the Chairman, Vice-Chairmen and Secretary for the new Board of Trustees for The Toronto Hospital were elected. However, all minutes continued to be bound identically and labeled as Minutes of Meetings for the Board of Trustees of the Toronto General Hospital. Minutes for the period of May 1848 to February 1866 are missing. Several items and appendices are also missing from the minutes for July, September and December 1997. A sheet listing the items missing from 1997 can be found in file 8.1.61 along with the minutes for January 1997.

The Audit Committee was formed as a standing committee of TTH Board of Trustees in October 1986. The mandate of the Audit Committee was to meet with the auditors and Board of Trustees at least twice a year to review and make recommendations on the external and internal audits, annual financial statements, and auditor reports. The Committee also made recommendations to the Board of Trustees regarding the annual appointment of auditors. Chairs of the Committee included David A. Lewis (1986-1989), Naju Shroff (1990-1993), and David Bawden (1994-199?).

The Clinical Quality Assurance Committee was formed as a standing committee of TTH Board of Trustees in October 1986. The Committee was responsible for encouraging and assisting all clinical, non-clinical, and diagnostic departments in the development and implementation of quality assurance mechanisms. The Committee regularly heard quality management reports from various departments to determine the effectiveness of quality mechanisms. The Committee also presented an annual report to the Board of Trustees detailing its assessment of quality assurance standards at TTH. The Quality Assurance Committee usually met once a month. Chairs of the Committee included Dr. A.A. Scott (1986-September 1990), David Bawden (September 1990-August 1993), and Beverley McParland (September 1993-199?).

The Finance Committee was formed as a standing committee of TTH Board of Trustees in October 1986. It was responsible for studying the annual budget and making recommendations to the Board of Trustees for approval of the budget. The Committee reviewed the financial statements of the hospital prior to each regular meeting of the Board of Trustees and presented its findings to the Board. The Committee also played an advisory role in recommending and reviewing the types of insurance carried by the Hospital and advising the Board about donations, endowments, investments, and other funds of the Hospital. Chairs of the Committee included D.B. Weldon (1986-July 1991), Fred Eaton (September 1991), and Jordan G. Sullivan (October 1991-199?).

A joint Hospital Relations Committee was formed on December 1, 1910, as a result of an agreement between the Trustees of the TGH and the Governors of the University of Toronto (Schedule 1 of The Toronto General Hospital Act, 1911) whereby the Hospital became a site for the clinical instruction of the University medical students. The Committee has alternately been called the Joint Hospital Relations Committee or the Joint Committee on Hospital Relations, and by 1981 it was referred to as the Joint Hospital/University Relations Committee.
The Committee was given the responsibility of supervising medical staff appointments.
<blockquote>The rights of the University as to clinical instruction were defined by a special Act, and a Joint Relations Committee arranged to make all appointments, which were for one year. A Head of Service, in the University, becomes ex-officio head of a Service in the Hospital (Clarke 126).</blockquote>
Thus, for example, the head of the University Department of Medicine was cross-appointed as TGH Physician-in-Chief. Prior to the establishment of this Committee, the responsibility of staff appointments had rested with the Hospital’s Board of Trustees.
The Committee was to consist of 8 members: 4 appointed by the Board of Trustees of the Toronto General Hospital and 4 appointed by the Board of Governors of the University of Toronto. The first members were Joseph Flavelle, P.C. Larkin, D.A. Wilkie and Alderman McCarthy (appointed by the hospital), and the Chancellor, Sir William Meredith, President Robert Falconer, Sir Charles Moss and the Rev. Dr. D. Bruce MacDonald (appointed by the University). Meeting minutes indicate that the Superintendent and Secretary of TGH were also present at meetings, though this was not stipulated by the Act. There were at least 3 meetings a year. By the 1920s meetings were less frequent with one meeting per year.
In addition to nominations for appointments to the medical staff of TGH, by the 1950s the Committee also discussed matters relating to the Pension Plan of the University, amendments to by-laws regarding retirement of staff, and staff retirements and resignations. Over time, Committee meetings included discussion of other Hospital/University agreements, progress reports on matters of relevance such as hospital mergers, establishment of endowed chairs, and education development plans. By the 1980s meetings also included discussions regarding the states of other Health Sciences Disciplines such as Social Work and Nursing.

After the merger of TWH and TGH in 1986, a Chief Operating Officer was appointed for each division. In 1989, the Board of Trustees decided to merge these two positions into the Office of the Chief Operating Officer/Senior Vice-President of TTH. The official designation of this office was changed to Chief Operating Officer/Executive Vice-President in 1992. The Chief Operating Officer reported to the President/Chief Executive Officer. The COO was chair of the Operations Committee and also oversaw various operational activities of the hospital including development of the hospital operating plan, human resources and labour relations, quality assurance standards, operational reviews of the hospital departments and the accreditation process. The COO also participated in long-range planning initiatives.

Series consists of the correspondence, reports, and committee minutes from the Office of TTH Chief Operating Officer/Executive Vice-President between 1985 and 1995.
The records were stored together in a filing cabinet and were arranged in alphabetical order with the exception of files 11.147-11.154 which were stored in binders on top of the cabinet. These files can be found listed at the end of the series with notes indicating the name of the original binder.
File 11.1.15 includes a 1993 report from the TTH Committee on the Status of Women. This file also contains a consolidated action plan based on reports from the Committee on the Status of Women, the Committee on Access for Persons with Disabilities, and the Task Force on Racial Equality.
File 11.1.43 includes a report entitled “Present Status and Future Directions of the Department of Obstetrics and Gynaecology at The Toronto Hospital” dated December 1990.
File 11.1.47 includes presentations from the departments of General Internal Medicine, Obstetrics/Gynaecology, Trauma, Emergency Medicine, Surgery, Opthalmology, and Psychiatry to the Long-Range Planning Committee.
File 11.1.48 includes a report from the Department of Otolaryngology to the Long-Range Planning Committee.
File 11.1.50 includes a report entitled “Consolidated Rheumatology Planning for The Toronto Hospital” dated 1989. The file also includes a “Toronto Western Master Plan Update” prepared by Karlsburger Companies which is dated June 1, 1988.
File 11.1.52 includes 1986 minutes from the Master Plan Steering Committee.
File 11.1.60 includes a report of the final recommendations of the Monitoring Equipment Committee which is dated December 1993.
File 11.1.63 contains a report from the Multicultural Health Advisory Committee which is dated May 1, 1995. File also includes a report entitled “External Review of the Biochemistry Department” dated September 1994.
File 11.1.66 contains a report on the Pettit Endowment Fund (1983-1987) dated December 1987.
The original file titles for files 11.1.33 to 11.1.37 included the full names of patients. In order to ensure compliance with Ontario’s Personal Health Information, 2004 (PHIPA), the full names have been replaced with initials.

The Department of Strategic Planning was established in November 1987 and was responsible for the creation and implementation of post-merger strategic plans for TTH as articulated in the planning model entitled Vision 2001. A possible predecessor to the Strategic Planning department was the department of Program Planning which existed for less than a year (January 1987 to November 1987) with Vytas Mickevicius serving as the Director. Michael J. McCartney was the Corporate Director of the Strategic Planning department from November 1987 to November 1989. The department was terminated on November 30, 1989.
One of the main responsibilities of the Strategic Planning Department was the creation of a post-merger strategic plan for TTH which resulted in Vision 2001, a planning model tabled at an administrative think-in in January 1989. The department then prepared an implementation plan for Vision 2001 which was presented to TTH Board of Trustees for approval in late 1989. Strategic Planning worked closely with departments and their respective planning committees to help facilitate planning efforts and ensure compatibility with the goals outlined in Vision 2001. The Corporate Director served on many planning committees including the Planning and Priorities Committee of TTH Board of Trustees.

The Executive Vice-President reported to the President/CEO. In 1992, the position of Executive Vice-President was merged with the position of Chief Operating Officer. The Executive Vice-President oversaw the administrative functions of the hospital including finances and internal audits, construction and planning, and human resources. At different points in time, some or all of the vice-presidents of the various hospital directorates reported to the Executive Vice-President. William Louth served as Executive Vice-President from 1986 to 1992.

On April 28, 1998, the TTH Auxiliary - General Division and the TTH Auxiliary - Western Division joined to form The Toronto Hospital Auxiliary. The Toronto Hospital Auxiliary was dissolved on September 30, 1998.
The members of the Auxiliaries raised money to support initiatives such as the purchase of medical equipment, building and renovation projects, medical research, and bursaries and awards for students. Money was raised through fundraisers such as bake sales, gala events, and hospital gift shops. Auxiliary members also volunteered their services in different areas of the hospital including the gift shops and patient libraries as well as visits with patients.
The auxiliaries created many committees to carry out specific activities and functions. Some of these committees include the Sewing Committee, Library Committee, Concert Committee, Archives Committee, and the Nominating Committee.
The TGH Auxiliary was founded in November 1911 as the Ladies’ Committee, an association of volunteers committed to providing comfort and assistance to Hospital patients, and to support the work of the TGH Social Service Department. Dr. Helen MacMurchy served as first chair of the Committee (1911-1913). The Committee was known successively as the Social Service Association, which was part of the Social Services Department of the hospital (1921-1953); the Women’s Auxiliary (1954-1976); the TGH Auxiliary (1977-); and the TTH Auxiliary - General Division (1992-1998).

Series contains published histories of the TTH auxiliary and its predecessor bodies. Series also contains two meeting gavels as well as pins handed out to members in recognition of their years of service.
The Auxiliary’s recognition pins were originated and established by the committee of Terry Globe, Margaret Hicks and Ruth Johnson in 1989. The pins were given out for the first time on Volunteer Night, October 4th, 1989, and afterwards to all volunteers with five or more years of service. The first lot of pins (approximately 200 – value approximately $2, 300.00) were paid for by the Cradle Pictures baby photo company and given as a gift to the Auxiliary.
